| Job Description: |
- Identify /Manage/ referral high risk cases
- Ensure Health education sessions especially.
- Record all health activities & report in line with set indicators
- Conduct monthly coordination meetings at the facility with internal and external team members
- Analyze monthly/ Quarterly data & improve clientage ratio at the facility to achieve set targets
- Ensure submission of reports daily/weekly/monthly as per reporting requirements
- Ensure & execute Infection prevention & Hospital waste management protocols at facility adopted at all levels
